It’s Re-Enrollment Time
All food program enrollment forms expire on August 31st. You should have received your re-enrollment package with all the instructions on how to re-enroll your day care children just after the first of the month. If you have not received your packet call Horizons. 1920-826-7292.
Re-enrollments must be updated, parent signed and dated then returned to the Horizons office by September 16th. Please call if you have any questions.
