This year Horizons will only be offering an in-home study packet to fulfill the required training.  We developed an PDF questionnaire you can fill in and electronically sign and email back to us for your convenience.

 

  1. Instruction Page:  Explains what you need to do.
  2. Required Training Booklet:  An informational resource to help you answer the questions.
  3. Home Study Questionaire:  This needs to be filled out and returned to Horizons.

 

If you are unable to open the resources let us know and we will send a hard copy of the questionnaire.  Please return at your earliest convenience and no later than July 31st.
